Consent and release for a child
I am the parent or legal guardian of the child named above. I give my child permission to climb, boulder and take part in classes, parties and other activities at Basecamp Climbing Gym, and to use its walls, ropes, equipment and training area.
The risks. Climbing and bouldering are physical activities with real risks. My child may fall from a wall or a hold, or while being lowered, and may land on padding, on the floor or on another person, and may be hurt by a fall, by a hold or piece of equipment that fails, or by another climber. Injuries can range from bruises, sprains and broken bones to serious injury, paralysis and death. I have talked with my child about these risks. I accept them for my child.
Rules. I’ll make sure my child knows to follow the posted rules and staff directions. My child will not belay, lead or use an assisted-braking device unless staff have checked them out on it, and will stay off the climbing floor and mats when not climbing. I understand staff may ask my child to stop climbing or to leave.
Supervision. During open climb, a child under 12 must have an adult in the building, and a child under 8 an adult on the climbing floor. In classes and parties, staff supervise the activity; I’ll pick my child up on time.
Release. As far as the law allows, I release Basecamp Climbing Gym from any claims that I or my child may have for injury, illness, death, loss or damage that come from my child taking part, including those caused by ordinary negligence, except those caused by its gross negligence or willful misconduct.
Emergencies. If my child needs emergency care and I can’t be reached, I allow staff to call for it, and I’m responsible for its cost.
How long it lasts. This agreement covers every visit, class, party and event for one year from the day I sign it.
Other terms. If a court finds part of this agreement can’t be enforced, the rest stays in effect. In this agreement, “Basecamp Climbing Gym” means the gym named on this website and its owners, employees, coaches and volunteers.
I am the child’s parent or legal guardian and I’m 18 or older. I have read this agreement, I understand it, and I sign it of my own free will.
